General Consent

I confirm that I am the legal owner or authorized agent for the dog listed above and authorize grooming services to be

performed. I understand grooming involves handling, equipment, and sharp tools, and that some risk is inherent even with

proper care.

Health, Behavior & Disclosure

I agree to disclose all known medical conditions, injuries, behavioral concerns, sensitivities, or grooming history prior to

service, including but not limited to seizures, arthritis, heart conditions, anxiety, aggression, skin issues, or recent surgery.

Puppies

I understand puppies may have limited tolerance due to age or inexperience. Grooming sessions may be shortened or

modified. Cosmetic perfection is not guaranteed; safety and positive experience take priority.

Senior & Special Needs Dogs

I acknowledge that senior dogs or dogs with medical conditions may experience increased stress or fatigue. Grooming may be

adjusted or discontinued if continuation could compromise the dog’s well-being.

Matted Dogs

I understand matting may require shaving or humane removal and may result in redness, irritation, clipper burn, nicks, or

post-groom sensitivity. I authorize necessary coat removal and accept that grooming outcomes may differ from my request.

Stress, Injury & Emergency Care

I understand some dogs may react unpredictably to grooming. Danielle’s Pet Grooming is not responsible for stress-related

illness, injury, or pre-existing conditions that surface during or after grooming. In an emergency, I authorize veterinary care at

my expense if I cannot be reached.

Release of Liability

I release and hold harmless Danielle’s Pet Grooming, its owner, and staff from any claims or liability arising from grooming

services.

Acknowledgment

I have read and understand this waiver and agree that Humanity Over Vanity guides all grooming decisions.
